Stool Ova & Parasite Exam: New Order Criteria

Effective September 15, 2021, Spectrum Health’s Epic will contain new ask-at-order questions to help improve clinical decision support and appropriate utilization of stool ova and parasite (O&P) testing. O&P testing may be used to diagnose several parasitic infections, though the staining of stool smears and their microscopic review is very labor intensive for laboratories. Historically, O&P testing has been widely ordered for patients with diarrhea, however, there are now other testing options able to detect the most common pathogens associated with community-acquired diarrhea. While Enteric Pathogens PCR and Giardia/Cryptosporidium testing is more appropriate for the identification of common bacterial/viral and parasitic pathogens, respectively, O&P testing should be reserved for patients with specific exposure or immune status criteria.

The Pharmacy and Infectious Disease Stewardship Committee has endorsed the following order criteria for O&P testing. If any of these criteria are met, then then order can be placed.

♦ Past foreign residence or recent foreign travel followed by at least 2 weeks of diarrhea.

♦ Immunocompromised status.

♦ Unexplained microcytic anemia or peripheral eosinophilia.

♦ Unique exposure (daycare, MSM, waterborne outbreak, etc.)

Note: If no criteria are met, cancel the order and consider Giardia/Cryptosporidium Screen (LAB258) or Enteric Pathogens PCR testing (LAB3618).

Appropriate Specimen Labeling

Spectrum Health Regional Laboratory is committed to the safety of our patients, and the accuracy and quality of the testing we provide.  In order to do that, we need your help to ensure that patient specimens adhere to our internal policies and quality checks.

THE DETAILS

To that end, we are asking for your help in the process of labeling specimens before they are sent to our laboratory.  It is our policy at Spectrum Health Regional Laboratory that the routine labeling of laboratory specimens MUST INCLUDE FULL LEGAL NAME and DATE OF BIRTH, and if possible, additional identifiers such as medical record number, specimen number, etc. If this information is missing, incomplete or incorrect, the specimen is considered a mislabeled specimen, and will be canceled upon receipt.  (The only exception to this is “precious specimens,” ex. cerebrospinal fluid or surgical specimens, which can sometimes be given an opportunity for relabeling after correct identification).

If a patient specimen is labeled with a NICKNAME instead of the FULL LEGAL NAME, this specimen would be considered a mislabeled specimen and have to follow the above process, which will delay patient care and/or employee eligibility to work.

WHAT WE NEED YOU TO DO

Please review this notice with your collecting/labeling staff as soon as possible to ensure proper labeling practices.  Though not new, this policy has been reviewed by our team members in central specimen processing, and will be strictly adhered to going forward.  Please feel free to forward this notice to anyone involved in the collecting or labeling of laboratory specimens that are sent to Spectrum Health.

Laboratory Order Validity Update

Spectrum Health Laboratory has updated its policy regarding expected date. For those providers that send an expected date of draw with your electronic order to our laboratory, there was a change on April 24, 2019 for Epic Providers and there will be a change on May 13, 2019 for EMR interface offices. From these dates forward, if you send us an expected date of draw, our phlebotomists will draw up to 14 days before your expected date and anytime thereafter until the order expires.  If we do not receive an expected date of draw, we will treat the order as ready to collect from the day it arrives till one year out, when it expires.  All Laboratory orders, including standing or future orders, are valid up to 400 days unless otherwise specified. Faxing Orders to Outpatient Laboratories

Spectrum Health Regional Laboratory now has ONE centralized fax number to which you can send your lab orders! This means no more searching for the fax number for a particular draw site for your patient. We will enter the faxed orders into our system, making them available to ALL of our draw sites; so your patient will have access to their order at their favorite draw site and the added convenience of going to any draw site that they choose!

 *If you are faxing an order for a patient that plans to go to a draw site immediately, please write “Patient En Route” on the order, so that it may be prioritized.

 * Please note: Specimen add-on orders (additional tests for specimens in-lab) are still faxed to 616-267-2751, as stated on the add-on form.
