Update: Antinuclear Antibody (ANA) Testing Options

In mid-September 2017, Spectrum Health Regional Laboratory Immunology department in collaboration with Spectrum Health Adult and Pediatric Rheumatology have made some adjustments to antinuclear antibody (ANA) testing (see below) options.

1. ANA Screen Reflex (Test Code: 116; Epic Code: LAB723) test will be DISCONTINUED.

Previously, A positive result for this test would have triggered testing for multiple ANAs. (Anti-dsDNA, anti-Sm, anti-RNP, anti-SSA, anti-SSB, anti-Scl70, anti-centromere, anti-Jo-1, anti-Sm/RNP, Ribosomal P, and Chromatin).
The individual ANAs will be still be available to order as single tests.

2. Result comments will be added to the remaining ANA orders (ANA Screen and ANA HEP2 (IFA)).

a. ANA Screen (Test Code: 184; Epic Code: LAB147)
New Comment: IF POSITIVE, recommend ANA HEP2 as next step/confirmatory testing. Positive specimens are saved for 30 days. Fax requests to 616-267-2771.

b. ANA HEP2 (IFA) (Test Code:1122; Epic Code: LAB3540)
New Comment: IF titer >= 1: 160, please order the individual ANAs (Anti-dsDNA, anti-Sm, anti-RNP, anti-SSA, anti-SSB, anti-Scl70, anti-centromere and/or anti-Jo-1) that are consistent with the disorder mirroring the clinical impression.

These changes and recommendations were collaboratively designed to improve utilization of ANA testing.
